Flagship’s ProFound partners with Gates Foundation in $35M preeclampsia target discovery pact

The Gates Foundation has entered a partnership worth up to $35 million with ProFound Therapeutics, a Flagship Pioneering biotech company, to identify the first viable drug targets for preeclampsia — a dangerous pregnancy complication that remains one of the leading causes of maternal and infant death worldwide, particularly across the developing world where clinical care infrastructure is weakest.

Under the terms of the agreement, ProFound Therapeutics will receive an initial $20 million to begin the research programme, with a further $15 million unlocked if the collaboration hits defined experimental milestones. The biotech will deploy its proprietary proteomics data library — built from proteins in what researchers call the “dark proteome,” a class of biological material invisible to conventional detection methods — to scan placenta tissue from women with and without preeclampsia side by side, searching for the biological signals that distinguish one from the other.

Those signals, once identified, become the starting point for drug development. ProFound Therapeutics plans to run candidate targets through an agentic AI tool to map their function before validating findings through laboratory experiments — a pipeline that combines large-scale data analysis with direct biological testing in a sequence designed to accelerate discovery in a disease that has historically resisted it.

The case for urgency is not subtle. Preeclampsia, which manifests as high blood pressure during pregnancy and can progress to fatal outcomes for both mother and child without medical intervention, is poorly understood at the biological level despite being relatively common. ProFound Therapeutics CEO John Lepore described the knowledge gap plainly, noting that despite the condition’s significance as a cause of maternal and fetal death — especially in contexts where acute medical care is not accessible — very little is known about how and why it develops.

That knowledge gap is partly structural. According to Herbert Virgin, who leads the Gates Foundation’s AI-Enabled Cures Frontier Accelerator, only around 1% of global healthcare research and development is currently directed toward conditions that specifically affect women outside of oncology. The consequence of that chronic underinvestment is a pipeline so thin that even when science identifies a promising direction, the path from discovery to treatment routinely takes more than a decade — far too long for the women in low and middle income countries who bear the heaviest burden of the disease.

The Gates Foundation’s decision to deploy $35 million into target discovery rather than late-stage clinical trials reflects a deliberate upstream bet. Without established biological targets, the pipeline for preeclampsia treatments cannot exist. This collaboration is designed to create that foundation.

The deal extends a pattern of high-profile partnerships ProFound Therapeutics has assembled since its launch in 2022, following agreements with Novartis, Pfizer, and GSK across different therapeutic areas. The company’s internal development pipeline is focused on oncology, where Lepore said researchers have identified a set of novel proteins unique to cancer tissue that are structurally accessible for drug development. The Gates Foundation partnership operates alongside that internal work rather than replacing it, consistent with what the company describes as a deliberate strategy of pairing focused internal development with targeted external collaborations.

For the Gates Foundation, the investment represents a continuation of its long-standing emphasis on maternal and reproductive health outcomes in low and middle income countries, now extended into the frontier of proteomics and artificial intelligence as tools for closing the gaps that conventional pharmaceutical research has left behind.

The collaboration was initiated after a conversation between Lepore and Virgin roughly a year ago, during which both parties recognized that ProFound Therapeutics’ platform capabilities and the Gates Foundation’s AI-driven research agenda were complementary. Preeclampsia emerged from that conversation as the priority target, a condition with significant unmet medical need, biologically accessible tissue for study, and a patient population concentrated in exactly the communities the Gates Foundation exists to serve.
